A Cass County program is one of six outstanding programs recently recognized with Association of Minnesota Counties’ (AMC) 2012 County Achievement Awards for exhibiting excellence and innovation.

Cass County’s Public Health and Social Services Integrated Intake Model was recognized.

Cass County Health, Human and Veterans Services has achieved integration of the agency through development and implementation of an integrated model of intake between public health and social services. This integrated intake model was designed to improve customer service by having social workers and nurses provide agency intake calls for services for both divisions at the same time.

Through steps of planning, training pilot groups, communication and full implementation of staff, the integrated intake model was actualized. Most importantly seen since implementation is an integrated, collaborative and holistic approach to meet customer’s needs between divisions has been achieved instead of working in independent focused areas of work. Benefits have been for customers, staff and process.

2013 AMC President Joe Vene, a Beltrami County commissioner, presented the awards at the association’s annual conference Dec. 10 in Minneapolis. The recipients were nominated by their counties and selected by a committee of county officials, chaired by 2013 AMC Second Vice President Christopher Shoff, DC, a Freeborn County commissioner.
