Crow Wing County scores high on health plan audits
Crow Wing County scored 100 percent on two Elderly Waiver/Community Well Care Coordination audits recently conducted by Blue Plus and Medica.
The annual audit involves reviews of case plans and assessments of seniors living in their homes to make sure the county is in compliance with all requirements. Health plans reimburse the county for care coordination.
Crow Wing County employs eight care coordinators who serve approximately 630 people living in the community. The coordinators meet with people in their homes to discuss what services the individual could access in order to remain in an independent living setting.
Services most often recommended by care coordinators include: home care/nursing services, adult day care, Meals on Wheels, personal emergency response systems and other equipment, assisted living and respite to provide relief to caregivers.
“The results of this audit show our care coordinators are accurately and efficiently providing information and services to seniors so they can make informed choices on their own health care,” said Gwen Anderson, Health and Social Services Division manager.
