Essentia participates in initiative to improve patient experience when care is needed beyond hospitalization

Essentia Health-St. Joseph’s Medical Center and Clinics recognize the team effort it takes to care for those in the community and participate in a collaborative effort to improve the patient experience and care.

Stratis Health, based in Bloomington, in its role as Minnesota’s Medicare Quality Improvement Organization, is leading a quality improvement initiative to improve care coordination when patients are going from a hospital to home, home health care or nursing care setting. The project is funded by the federal Centers for Medicare & Medicaid Services (CMS).

Brainerd lakes area organizations participating in this initiative include Essentia Health; Comfort Keepers; Good Samaritan Society skilled nursing facilities Bethany, Pine River and Woodland; Good Samaritan Society Home Care & Hospice; Thrifty White Pharmacy; and the Central Minnesota Council on Aging. This group has come together as a team for learning, sharing and networking.

This team has access to quality improvement experts, evidence-based practices and patient feedback. They have identified community-specific causes for poor transitions, engaged community stakeholders as partners, and implemented evidence-based interventions.

“We want our patients and family members to have the best care experience in our community. We have focused efforts when they are our guest and a care team at the hospital or clinic is working with them. But it is very exciting to have our staff working collaboratively with those who continue this care at the patients’ home or nursing care facility,” said Kathy Miller, director of Quality Improvement and Assessment at Essentia Health-SJMC and Clinics.
